AFFIDAVIT

I, S S/o Sh. G R/o House No. X, Shalimar Garden, Extn-1, Sahibabad, Ghaziabad, UP-201005, do hereby take oath and solemnly affirm and declare as under:-

That I am citizen of India and residing at said above address.

That my actual and correct name is S according to my Govt. Employee Card, Aadhar Card, Pan Card & Driving License but wrongly mentioned as SA in my Railway Service Record.

That S and SA is the same and one person.

That I shall be known as S for all the purposes and in short name is SAM.

That I shall abide by all the rules and regulations.

That it is my true and correct statement.
